I received a note from Alan Riley at Statacorp that is of relevance to
this thread and have his permission to post an excerpt:

However, I did want to let you know that Stata can print SMCL
files in color. The default is to map everything to monochrome,
as you know.
Try the following sequence of things in Stata:
- open a Viewer looking at some SMCL file which has some
text in it which may be red, blue, etc.
- right-click on the Viewer and select Print...
- in the resulting Print dialog box, choose the printer you want and
click the Print button
- (here is the important step) you should now have an 'Output
Settings' dialog box pop up. Rather than clicking 'OK' in it,
click the 'Prefs...' button at the lower right.
- play around with changing the 'Color Scheme' in that dialog box.
I suggest 'Custom 2'--it is the same as the built-in 'Color'
scheme for printing SMCL files, but you can choose what color
is printed for each type of Stata output.
