I used stcox to evaluate risk of outcome x  associated with ever-never exposure to y.  I am now interested in a dose-response analysis for all number of doses of y.  Sometimes subsequent doses are received in a matter of weeks, sometimes they are received years after the first dose.  Should I create separate records for each period of follow-up at a given number of doses?  If I do this, is there any reason to maintain multiple-observation per subject data? Or can the records at each number of doses be analyzed as separate subjects?  I ask this last point because it is a HUGE dataset (> 10^6).
