Michael,
If you check out the repec page for ivreg2,
http://ideas.repec.org/c/boc/bocode/s425401.html
you'll see that several files will have to be downloaded and copied to
appropriate folders by hand: specifically, the ado, the sthlp, and the
mlib files.
Where you put them on your machine will depend partly on your setup, but
perhaps others on the list can advise on the best place in your
circumstances.
