I ran into this problem a lot using the latest Stata and -ice- commands. My only workaround was to dramatically simplify the impute equations--reducing the number of right-hand variables per each imputed var. I'm not satisfied with that solution, but it was the only way to kill that error.
I assumed in my case that the data were the problem--too small of an n, given k predictors per impute equation. Ultimately I abandoned Stata -ice- and went to a Bayesian approach for the imputations. Following that, we created a dataset that could utilize Stata's MI prefix.
I'm very curious what you learn and how you solve this in your situation David and I hope you can keep us posted??

Sounds like an error in the program. I would first make sure
that I have the most recent version of -ice- (the one on SSC
not the one from the SJ). So type in Stata:
ssc install ice, replace
If that does not solve your problem I would sent an email,
containing a dataset and a .do file that reproduces your
problem to Patrick Royston, the author of -ice-. Patrick is,
to the best of my knowledge, not a member of the Statalist,
so you need to contact him directly.
