Thank you Maarten and Martin for your replies. 1) Maarten, Stata did 'close' (not just stop responding) with a not so informative error message: "Stata has encountered a problem and needs to close...". I will contact Stata tech support in due course as you recommended. 2) Martin, "bs_secondstage.ado" draws samples of individuals within clusters chosen at stage 1, calculate the mean cost and mean quality-of-life measure (two outcomes here) for the treatment and control groups. These summary statistics are stored as scalars. These scalars are subsequently used for calculating something called the incremental net benefit which in turn is stored as a scalar. The code ends.
