I have always wondered how this happens. Are the ado-files more easily
-update-d than the executable? I always have Stata check the -update- status
and do its thing, and sometimes it prompts me to allow "administrative
tools". Is that what prevents users from properly updating the whole thing?

It is fairly disastrous to get the executable out of sync with the
ado-files. You should either update both or update neither, but under no
circumstances create this sort of asynchrony. It has often been reported on
Statalist as the source of serious problems with Stata installations.
