It's not Stata, but Unix and DOS versions of "Programs for Computing Group Sequential Boundaries Using the Lan-DeMets Method" are available at http://www.biostat.wisc.edu/landemets/ (a Google search will turn up more sites).

I know of none.  When I was on a DSMB a couple of
years ago, I used EAST from Cytel software.
