The CMS website has public use data files of both the ICD9 Procedure codes and HCPCS. I haven't seen a cross walk but it might be worth checking out.

Subject: st: ICD9 Procedure codes
Is anyone aware of a publicly available file that provides some sort of
crosswalk from ICD9 Procedure codes to CPT/HCPCS Codes?
Thanks,
Michael
